These Ambetter plans do not require a referral from a primary care provider (PCP) for routine podiatry visits at our practice. However, certain services may still require prior authorization, depending on the patient’s specific plan and the type of service being performed.
Referral & Authorization Policy
Some insurance plans require a referral or prior authorization before your visit. These rules are set by your insurance company, not our office.
Checking this ahead of time helps prevent:
- Denied claims
- Unexpected out‑of‑pocket costs
- Delays in care
Your Responsibility
Patients are responsible for confirming whether their insurance plan requires:
- A referral
- Prior authorization
- Any special documentation
If a Required Referral or Authorization Is Missing
If a required referral or authorization is not obtained by you or by our office, a fee equal to 25% of the standard office visit charge will apply.
This policy helps ensure your visit remains covered and reduces the risk of surprise bills.
